Reshaping Care for Older People -Change Fund Update

As part of the Scottish Goverment's, Reshaping Care for Older People Programme, the Change Fund was introduced to fund a shift in the balance of care from institutional to primary and community settings.  The principal goal is to care for older people by linking in with family and community support networks and building effective partnership working across the statutory and non-statutory sectors.  Helping older people to be cared for at home or in a homely setting.
